Client Agreement
This agreement is with Deep Insights Coaching.
Your psychotherapist, transformational coach and mentor is Bill Petrie.
Bill Petrie
Bill has been working as a Clinical Psychologist, Psychotherapist, Transformational Coach and Mentor for more than thirty years. He currently works online with clients internationally.
Bill’s qualifications include: M.A. in Clinical Psychology (UCT); Diploma in Cognitive Therapy (Oxford University); Diploma in Advanced Transpersonal Psychotherapy (CCPE, London) & Diploma in Psychotherapy Supervision (CCPE, London).
Bill is deeply committed to providing a highly ethical and professional service.
The professional role that Bill adopts with you depends in part on your country of residence and its laws.
To give five examples:

For those who are resident in the United Kingdom, Bill can legally adopt the roles of Psychotherapist, Coach or Mentor.

For those who are resident in France, Bill can legally adopt the roles of Counsellor, Coach or Mentor.

For those who are resident in Germany, the USA or Canada, Bill can legally adopt the roles of Coach or Mentor.

For South African residents, Bill can legally adopt any of the above roles as well as that of a Clinical Psychologist.
Bill’s sole professional registration is as a Clinical Psychologist with the Health Professions Council of South Africa (HPCSA) where he is registered as William Petrie and his registration number is PS 0026670. The HPCSA has no jurisdiction over clients who reside outside of the borders of South Africa.
Session Duration
Sessions are 50 minutes long and each session costs £135.
There will be an increase in costs each year in January.
Information Deep Insights Coaching collects about you and how it is used
In the work with Bill, basic personal information will be collected for contact, contract and identification reasons.
During your sessions, an assessment of your psychological health may be completed, and notes may be taken during sessions. These will include personal and sensitive details about your life. Sessions may also be recorded.
We may also collect user and technical data (i.e. data about how you use our website and any online services together with any data that you post for publication on our website or through other online services), marketing data (that includes data about your preferences in receiving marketing from us and our third parties and your communication preferences) and customer data (i.e. data relating to any purchases of services such as your name, title, billing address, delivery address email address, phone number, contact details, purchase details and your card details). This data may be used to facilitate the business aspect of the practice.
All of your information will be stored on a password-protected, encrypted drive.
The recordings, the assessment and other notes are used solely for the delivery of a therapy service to you.
Confidentiality
Deep Insights Coaching takes confidentially extremely seriously.
The only situations in which we may break confidentiality are:

If we are required by a court of law to divulge your information

If Bill Petrie believes that you or someone else is at serious risk

If, in the extremely unlikely event that you bring legal action against Deep Insights Coaching, any and all of your information may be used by Deep Insights Coaching in defending against those legal proceedings.
Sharing of data
Your information may be shared with a supervisor who is similarly bound by the ethics of confidentially. If Bill discusses you with a supervisor, he will only refer to you by your first name.
Personal data may also be shared with service providers (who provide IT and system administration services) or with auditors, bankers, lawyers and insurers. In these instances, we require all third parties to whom we transfer your data to respect the security of your personal data and to treat it in accordance with the law. We only allow such third parties to process your personal data for specified purposes and in accordance with our instructions.
Other than in the case of those exceptions outlined above under ‘Confidentiality,’ Bill will always explicitly ask your consent before sharing any of your data with 3rd parties. If data is sent to 3rd parties with your permission, Bill will send the data securely.
International Transfers
We are subject to the provisions of the General Data Protection Regulations that protect your personal data. Where we transfer your data to third parties outside of the EEA, we will ensure that certain safeguards are in place to ensure a similar degree of security for your personal data.
Session Payments
All sessions are payable in advance.
After the first 50-minute session, bundles of four sessions are purchased at a time.
Payments may be made to a UK bank account or you can opt to pay via a credit or debit card. If you would prefer this latter option, please let Bill know and we will generate a payment link for the same.
Receipts/invoices are available on request.
Cancellations
48 hours’ notice is required to cancel your session without incurring a charge.
If your appointment is on a Monday, you are required to cancel on Friday. If these conditions are met, there will be no charge. If they are not met, you will be charged the full fee for the session. There will be no negotiation on this issue. If you do not attend the session, you will be charged the full fee for the session.
You have the right to end the work with Bill at any time. Any funds in credit will be returned. Bill also retains the right to end the work should he deem it wise to do so.
Your rights
You have rights relating to the information Deep Insights Coaching holds.
You have the right to verify the accuracy of the information or to ask for the information to be supplemented, deleted, updated or corrected.
You have the right to request a copy of the information that Deep Insights Coaching holds about you. If you would like a copy of some or all of your personal information, please email or write to Deep Insights Coaching via the contact details stated in this agreement. Information will be provided to you within 30 days.
You have a right to request the transfer of your data to another individual or company.
We want to make sure that your information is accurate and up to date so please inform us timeously of any changes in your personal information.
How long we keep your information for
Your information is kept for the time necessary to provide the service requested, however, we will hold your details and session notes for a period of up to 7 years following the end of treatment to comply with legal obligations that are placed upon him by our insurers. After this date, all data that has not been anonymised will be securely deleted.

Declaration by you, the client
I acknowledge that I have had the opportunity to read this agreement carefully and I have addressed any questions or concerns that have arisen from it with Bill Petrie.
I have read Deep Insights Coaching Privacy Policy (www.deepinsightscoaching.com/privacy-policy) and I have understood the information in that Policy.
I understand that I am personally responsible for the payment of my sessions.
Given the conditions outlined in both this document and the Privacy Policy referred to above, I willingly give my informed consent to participate in Psychotherapy, Coaching and/or Mentoring with Bill Petrie.
